Pay Range
$22.50 - $33.75
Delivers quality early intervention service coordination (targeted case management) to infants and toddlers ages 0 to 3 years and their families. Ensures that infants and toddlers referred and/or enrolled in the applicable state Infant-Toddler Program (applicable state ITP) receive service coordination provided in accordance with and in compliance with Federal and State regulations, ensuring parents rights in the program. Provides targeted case management, resources, and referrals to appropriate community programs as needed within the six county catchment area of the Winston-Salem Children's Developmental Services Agency (W-S CDSA). Meets applicable state ITP mandated timelines and documentation for enrollment, individualized family services plan (IFSP), intervention services as needed, transition planning, and transition upon the child's third birthday.
EDUCATION/EXPERIENCE
Graduation from an accredited college or university with a Bachelor's degree in one of the following: Birth-Kindergarten Education, Early Childhood Special Education, Special Education/Education, Child Development, Human Development-Family Relations, Family Studies, Family and Consumer Sciences, Psychology, or Social Work required. Two years of post-baccalaureate full-time experience as an early intervention Developmental Specialist or equivalent at a applicable state CDSA or similar early intervention program in another state required; or, Master's degree and one year of full-time experience as an early intervention Developmental Specialist or equivalent at a applicable state CDSA or similar early intervention program in another state.
LICENSURE, CERTIFICATION, and/or REGISTRATION
Infant-Toddler Family Specialist Certification preferred or must be eligible for Infant Toddler Family Specialist certification and maintain Infant Toddler Family Specialist certification through continuing education annually.
Valid NC driver's license.
ESSENTIAL FUNCTIONS
1. Provides targeted case management/service coordination based on compliance with Federal law IDEA Part C, and state program applicable state ITP guidance governing Infant-toddler early intervention, Service Coordinators ensure family procedural safeguards and entitlements, meeting Federal and State required timelines, compliance indicators, and paperwork for all service coordination activities.
2. Responsible for explaining applicable state ITP early intervention to families and ensuring parents rights for eligibility, the child?s developmental strengths and needs and services within applicable state ITP. Explains enrollment based on eligibility status to parents, referral sources, and community professionals as needed.
3. Responsible for gathering information from the family, beginning a family directed identification of family needs, strengths, priorities, concerns, and resources.
4. Participates as member of a team in conducting developmental evaluations or child assessments determining program eligibility, and preparing, developing, monitoring, and revising Individualized Family Service Plans (IFSP).
5. Assists families in accessing other services and resources as needed. This includes referrals and arranging service provision with follow-up as needed. Service Coordinators work with other CDSA staff and/or network service providers to meet identified child and family needs/outcomes.
6. Assists families when child is two years of age to prepare for transitioning from the applicable state Early Intervention program when the child turns three years.
7. Performs other related duties as assigned by the supervisor and assures that record keeping and documentation at all times is compliant with applicable state ITP and W-S CDSA and WFBH program requirements.
SKILLS/QUALIFICATIONS
Excellent and effective communication and interpersonal skills
Respectful and sensitive with respect to a diverse and multicultural population of children, families, and co-workers
Skills in problem-solving, mediation, conflict resolution and teamwork are essential for collaboration as a CDSA team member, as well as for work with families and community agencies
Computer literacy and experience with Microsoft Word and Microsoft Office is a requirement for all W-S CDSA positions
Thorough knowledge of the concepts, practices, and techniques in the field of Early Intervention, infant-toddler assessments and development of IFSPs
Requires considerable knowledge of child development, family dynamics, developmental disabilities, and factors which place infants/ toddlers at risk for developmental delays
Skill in working with young children and their families in collaborating with parents, co-workers and other professionals. High degree of soundness in judgment and accuracy in eligibility determination
Ability to apply and comply with Medicaid standards, IDEA policy requirements
Familiarity with applicable state ITP policies and procedures
Experience writing treatment/or educational family service plans Experience working with community support systems and resources
Ability to use the guidelines, rules, and procedures to effect job duties and responsibilities
WORK ENVIRONMENT
